Disclosure: Searchmaxxed wrote this list and is in it, and we put ourselves first.
The best healthcare SEO agency for a private clinic shows results from practices like yours, knows the patient-data rules and names its first pages. For a practice that wants to be the name patients hear in Google and in AI answers, our pick is Searchmaxxed. This is our own selection, not an award.
Seven more are worth a call: Cardinal Digital Marketing, Intrepy Healthcare Marketing, Healthcare Success, Thrive Internet Marketing Agency, WebFX, Coalition Technologies and Sagapixel. We read every agency's own healthcare SEO page on 3 November 2026, and each entry uses only what that agency publishes about itself.
In summary
Match the agency to your practice. Then put the same seven questions to each one before you sign anything.

How did we choose them?
We started from the agencies Google showed in the US on 3 November 2026 for "healthcare seo agency", "medical seo agency" and "medical seo company". Then we read each agency's healthcare page that same day. We judged all eight on five things:
- Healthcare proof they publish: a named practice, specialty or provider type, with a number attached. Results from roofers and software companies count for less here, because patients search and choose differently.
- Patient-data knowledge: whether the page shows it understands HIPAA, business associate agreements and tracking on booking pages.
- Local search depth: Google Business Profiles, location pages, provider pages and health directories, for one site or hundreds.
- Price and terms: what it costs and how long you're tied in, where the agency says.
- AI search: a published offer for Google's AI Overviews, ChatGPT, Gemini or Perplexity.
We quote every result as the agency's own claim, so this list measures only what each agency chooses to show the public. How happy its clients are is a different test, so call two or three and ask for references before you sign.
1\. Searchmaxxed: best for practices that want to be the answer in Google and AI search
Searchmaxxed is an AI-native search team. We make your practice the answer in Google and in the AI engines patients ask, from ChatGPT to Perplexity, Gemini and AI Overviews. We build and improve your website, pages and articles. Then we fix the technical SEO and earn your place in Google, AI Overviews and ChatGPT. We suit private clinics and specialist groups where one new patient is worth thousands.
- Where they are: We work with clients worldwide. You talk to the people doing the work, with no account managers in between.
- What they sell: Service, condition, provider and location pages, technical SEO, Physician and MedicalClinic schema, Google Business Profiles, health directory clean-up, reviews and links.
- Speed and depth: Every person on our team works with AI agents. You get the reach of a much larger team with the focus of a small one.
- Proof they publish: Searchmaxxed's work has generated more than $15 million in revenue through search for its clients. We haven't published a healthcare case study yet, and we won't invent one.
- Pricing: Quoted after a free search analysis.
- Reporting: We track which searches and AI answers turn into enquiries and revenue. You own the site and the data from day one.
- AI search: We check which practices ChatGPT, Gemini, Perplexity and AI Overviews name for your specialty in your city. Then we go after the pages, listings and mentions those answers draw on.
- Ask them: Show me who ChatGPT names for my specialty in my city today, and the first three pages you'd build.
2\. Cardinal Digital Marketing: best for multi-location healthcare groups
Cardinal Digital Marketing suits a group adding locations through acquisitions, and says it has scaled local SEO "from 20 locations to more than 500". It calls itself "the #1 SEO agency in healthcare", and Clutch lists 95% of its focus as medical.

- What they sell: Local SEO at scale, citation management, location pages, technical SEO, link building, content and site migrations for merging brands.
- Proof they publish: A DSO with a "116% increase in lead volume", and "the country's largest mental health care provider" scaled to 500+ locations with an "82% increase in new traffic". An infusion center shows 356% growth in keyword rankings year on year.

- AI search: AEO, GEO and "AISO", with tracking across ChatGPT, Grok, Perplexity and AI Overviews, including sentiment.

- Ask them: How do you keep 40 location pages from competing with each other?
3\. Intrepy Healthcare Marketing: best for physician-led practices and MSOs
Intrepy Healthcare Marketing suits an orthopedic, spine or specialty practice that wants a healthcare-only team and a price it can see before the first call. Its named testimonials come from Tennessee Orthopaedic Alliance, Midlands Orthopaedics & Neurosurgery and Buckhead Medicine.

- Ask them: Which tracking do you remove from my booking pages, and who signs the BAA?
4\. Healthcare Success: best for hospitals and enterprise health brands

- AI search: An "AI-Ready SEO Audit", plus content it writes for AI Overviews to cite.
- Patient data: "HIPAA- and YMYL-informed SEO strategies."
- Ask them: What does the smallest engagement you'd take on look like for a 3-location practice?
5\. Thrive Internet Marketing Agency: best for SEO, reviews and paid media together
Thrive suits a practice that wants SEO, review building and paid search run by one large agency. Its healthcare page covers keyword research, links, content, technical SEO, franchise SEO and CRO, and it dates its work back to 2005.

- Ask them: How do you collect reviews without breaking patient privacy?
6\. WebFX: best for practices that want a published starting price and a big team
WebFX suits a practice or outpatient center that wants a starting price up front and a large bench of specialists behind it. It says it has "750+ digital experts" and "98,500+ hours of healthcare marketing experience".
- Where they are: Its healthcare page doesn't name an office. It claims a "30-year track record".
- What they sell: SEO, local SEO, PPC, web development, content and CRO, plus its own platform, RevenueCloudFX, and Nutshell CRM access.
- Proof they publish: Merrimack Valley Psychological, "2X Revenue in 6 months". Hurst Pediatric Dentistry, a "540% Increase in leads".

- Reviews: "1,100+ reviews", by its own count.
- Ask them: Who on the team has run SEO for a practice my size, and what did month one include?
7\. Coalition Technologies: best for cash-pay specialties and health ecommerce
Coalition Technologies suits a plastic surgeon, a cash-pay specialty or a health product brand that cares about organic revenue as much as bookings. It claims "up to a 1,577% increase in organic revenue for healthcare ecommerce and lead generation businesses".
- Where they are: Its healthcare page doesn't give an address, so ask where your team sits.
- What they sell: A six-step process from opportunity analysis to crawl fixes, keyword work, on-site experience and off-site PR and links.
- Proof they publish: Number-one rankings it lists for "pediatric dentist (Nevada)", "plastic surgeon (Kentucky)" and "women's health center (New York City)". Case studies cover Coast Medical and Medisys Health Network.
- Pricing: Not published. Its FAQ says a national campaign costs more than one doctor targeting a suburban market.
- AI search: Services for Google AI Overviews and ChatGPT, linked from the healthcare page.
- Reviews: 4.8 stars from "160+ reviews" on Clutch, by its own page.
- Ask them: Its FAQ says meaningful growth "typically" takes 8 to 12 months. What should I see by month four?
8\. Sagapixel: best for plastic surgery, med spas and hair restoration
Sagapixel suits a cash-pay practice that wants content in its own doctors' voices. It calls itself a pioneer of "video-to-blog": it films your doctor answering patient questions, then turns the video into an article.
- Where they are: Its SEO page doesn't give an address. It says Semrush rated it "a Top Healthcare SEO Agency".
- What they sell: Local SEO, technical SEO, on-page SEO, content and link building, plus web design, Meta ads, PPC and social.
- Proof they publish: An HRT and medical weight loss clinic with a "670% Lift in Organic Traffic in 3 Months". A plastic surgeon reached top 5 rankings in 6 months on a brand-new website, and a hair restoration clinic saw 84% more leads.
- Pricing: Budgets from $1,500 to $20,000 monthly, on month-to-month contracts.
- AI search: A dedicated SEO for AI service, and published research on how people choose local services in ChatGPT.
- Ask them: How many hours of my surgeon's time does video-to-blog need each month?
Does a healthcare SEO agency need to sign a BAA?
Yes, if the agency or its tools will receive protected health information from your website. HHS gives a plain example. A patient books through a clinic's website, and a tracker sends the appointment and IP address to a vendor. "In this case, the tracking technology vendor is a business associate, and a BAA is required."
That example lives on your booking pages, which is exactly where most SEO and ads tracking sits, from conversion tags to retargeting pixels. A form fill counts as a conversion. A thank-you page fires a pixel. Without care, both can carry patient details to Google or Meta.
The rule has a limit worth knowing. On 20 June 2024, a federal court in Texas struck out the part of the HHS guidance covering logged-out pages about conditions or providers. HHS says it is "evaluating its next steps", and its page was last reviewed on 26 June 2024. So the strictest rules now sit on your booking, portal and form pages, and a good agency should know which of your pages is which.
Good tracking matters for a second reason: you can't grow what you can't count, and plenty of practices count the wrong things. Here's what we found.
A client's four enquiry types never counted as conversions. Their event names ran 41 to 50 characters; GA4 cuts at 40. The 40-character "career" event counted every time. Nobody had noticed, because the events still showed up.
AI traffic needs the same care, because a bot can look exactly like a patient who came from ChatGPT. A client's GA4 showed AI-assistant visits jump from 354 to 6,050 in a month. Almost none of it was real. About 5,700 came in three one-hour bursts: same 1920x1080 desktop Chrome, all new users, no referrer, about 20 seconds each, landing on two pages. Real AI traffic was 385 sessions against 354 the month before, roughly flat.
A booking form with a long event name, or a bot spike counted as ChatGPT patients, would give a practice the same false picture. Ask every agency how it checks both.
Why does AI search matter for a medical practice now?
Because patients now use AI tools to choose a doctor about as often as they use Google. In rater8's survey of nearly 1,000 US patients, published 3 June 2026, 36% of recent doctor-searchers named AI tools as an influence. Google search came in at 34%, and doctor recommendations at 32%.
Those patients also trust what they read. AI Overviews were the most trusted part of Google for doctor research, at 37%, against 20% for blue links and 13% for the map pack. And 66% had met wrong information about a provider from an AI tool, yet 60% still trusted the AI summary without checking it further.
Rankings alone are half the picture. Ask what ChatGPT, Gemini and AI Overviews say about your practice today, and which pages and listings those answers lean on. We built our AI search optimization service around that question.
How should you choose a healthcare SEO agency?
Choose the agency that shows results from practices like yours, explains how it handles patient data and names the first pages it would build. Use these seven questions on every call.
- Ask for a practice like yours. A women's health result tells an orthopedic practice something. A nurse staffing result tells it far less. Ask for the specialty, the number of locations and the time it took.
- Ask who signs the BAA. If its tools touch booking, form or portal data, you need a business associate agreement first.
- Ask what changes on your booking pages. Which pixels come off? Which stay, and how do conversions still get counted?
- Ask for the page plan. One page per service, condition, provider and location beats one long services page. Which pages go live first, and why those?
- Ask about Google Maps. Google's help page says its local results are "mainly based on relevance, distance, and popularity". A good agency can explain how its local SEO and review work moves all three, location by location, for a group like yours.
- Ask who checks medical accuracy. Google gives more weight to strong E-E-A-T on health topics, which it calls "Your Money or Your Life" topics. Ask whether a clinician at your practice reviews each claim before it goes live.
- Ask what ChatGPT says about you. An agency selling AI search should show you today's answer for your specialty and city before you sign.
How much does healthcare SEO cost?
Published US healthcare SEO prices run from about $750 to $20,000 monthly. Most practice retainers sit between $1,500 and $5,000 monthly. Multi-location groups and hospital systems pay more.
| Source | What it publishes | Monthly ($) |
|---|---|---|
| Intrepy, starter SEO for solo physicians | Starting price | From 750 |
| Intrepy, multi-physician practices | Average retainer | 3,000 to 5,000+ |
| Sagapixel, healthcare SEO page | Budgets it works with | 1,500 to 20,000 |
| WebFX, healthcare marketing page | Custom plans | From 3,000 |
| WebFX, healthcare SEO cost guide | Typical agency retainer | 1,500 to 5,000 |
| WebFX, healthcare SEO cost guide | Local healthcare SEO | 300 to 2,000 |
| Thrive, proposal form | Lowest budget bracket on its form, not a price | 2,500 to 5,000 |
Sources: each agency's own page, read 3 November 2026, plus WebFX's healthcare SEO cost guide dated 28 October 2026.
Clutch adds a different signal, the smallest project each agency takes: $5,000+ for Cardinal, $10,000+ for Intrepy and $25,000+ for Healthcare Success. Those are project minimums, not monthly fees.
What moves the price? Three things: your number of locations, how hard rival practices fight for your specialty, and how much gets built each month. Then weigh it against one new patient. Where a single case or course of care is worth thousands, a few new patients can cover a monthly retainer.
Is Searchmaxxed right for your practice?
Searchmaxxed suits a private clinic, specialist practice or clinic group that wants to own its specialty's searches in Google and its name in AI answers. It suits you best if you want one team to build the site and pages, then report on the enquiries and revenue they bring.
It's a good fit if:
- One new patient is worth thousands to you, through a procedure, a course of care or years of visits.
- Patients in your city ask ChatGPT or Google's AI who to see, and your practice isn't named.
- You want the site, content and data in your own name from day one.
- You'd rather skip the account manager.
It isn't the right fit if:
- You need new patients this month to keep the doors open. Search builds over months.
- You want the cheapest retainer.
- You need an agency that can already show you a published healthcare case study. We can't yet, and Intrepy, Cardinal and Healthcare Success can.
- You run a hospital system and want a healthcare-only team that has done it for years. Healthcare Success and Cardinal publish that experience.

Frequently asked questions
What does a healthcare SEO agency do?
A healthcare SEO agency gets your practice found when patients search for care, then turns those searches into booked appointments. The work covers service, condition, provider and location pages, technical SEO and Google Business Profiles. It also takes in reviews, health directories, links and what AI tools say about you.
How much does healthcare SEO cost?
Published US prices run from about $750 to $20,000 monthly. Intrepy starts solo physicians at $750 monthly, and WebFX's 2026 guide puts typical agency retainers at $1,500 to $5,000 monthly. Multi-location groups pay more.
How long does medical SEO take to work?
Expect months, not weeks. Cardinal says SEO typically pays off in about 3 to 6 months. Intrepy says most practices see meaningful gains within 100 days, while Coalition puts meaningful growth at 8 to 12 months. Your specialty, city and starting site change that.
Does a healthcare SEO agency need to sign a BAA?
It does if the agency or its tracking tools receive protected health information, for example appointment details from your booking form. HHS says a tracking vendor in that position is a business associate and needs a BAA. Settle it before any tags go on booking pages.
Is medical SEO worth it for a small practice?
Yes, when one new patient is worth enough to carry the cost through the months before search results start to arrive. A solo specialist can win local searches with strong service pages, a complete Google Business Profile and steady reviews. Intrepy publishes a $750 monthly starter tier for solo physicians.
Should each doctor have their own Google Business Profile?
For a multi-location group, Cardinal advises one profile per location, not per provider, so providers don't compete with their own clinics. It suggests listing individual providers on Healthgrades, RateMDs and Zocdoc, while each doctor still gets a strong profile page on your website.
How do medical practices show up in ChatGPT and AI Overviews?
By being the clearest source. AI answers draw on pages they can read, listings that agree with those pages and reviews. Build a page for each service, line up your directory listings and reviews, then check which practices each engine names.
What's the difference between healthcare SEO and regular SEO?
Healthcare SEO works under stricter rules. Google holds health pages to a higher standard as "Your Money or Your Life" topics, and HIPAA governs patient data on your booking pages. Patients also read reviews before they book. In rater8's 2026 survey, 75% said they wouldn't book a provider rated below 4.0 stars.
